We accept most PPO group insurance.  Dr. Fassino is in network with Blue Shield of California, UnitedHealthCare, Multiplan, and Zelis.

We accept HSA cards for deductibles, co-pays, co-insurance, or non-insurance reimbursable procedures.

Blue Shield no longer covers chiropractic in  individual and family PPO plans (Covered California/Obamacare)

We are unable to send claims to these types of insurance plans or HMO plans.
